Support Coordination is the connecting link between all services. It assists the recipient in gaining access to services - not to deliver the services. Support Coordination is provided for the following populations:

Support Coordination utilizes the components of intake, assessment, service planning, linkage, monitoring / follow-up, reassessment and transition/closure. Eligibility is determined by requirements specific to each population. See each population for eligibility requirements.
