Cadence of Acadiana

The support coordinator will determine if the recipient is eligible for services by virtue of medical and/or other applicable eligibility requirements of the NOW Waiver population as defined by the Department of Health and Hospitals (DHH). The following are the specific eligibility requirements for NOW Waiver:

  • A severe chronic disability of a person, which is attributable to: mental retardation, cerebral palsy or epilepsy; or
  • Any other condition, other than mental illness, found to be closely related to mental retardation because this condition results in impairment of general intellectual function or adaptive behavior similar to that of mentally retarded persons, or requires treatment or services similar to those required for these persons; and
  • Which is manifested before the person reaches age 22; and
  • Which is likely to continue indefinitely; and
  • Which results in substantial functional limitations in three (3) or more of the following areas of major life activity: 1) self-care, 2) understanding and use of language, 3) learning, 4) mobility, 5) self-direction; and
  • The consumer must require and is unable to access services from multiple formal or informal services providers, except in the instance of consumers eligible for waiver services; and
  • The consumer is at risk of becoming homeless or in need of protection from harm due to environmental or life circumstances, need for supervision, or potential threat of abuse or neglect; or
  • The consumer has been institutionalized, is at risk of becoming institutionalized or would otherwise require Intermediate Care Facility for the Mentally Retarded (ICF/MR) level of care.
